Provision of Service

Provision of Service for accessing Arts 4 Wellbeing, requires you to be referred by a statutory agency example e.g. Social Services, GP, CPN, Shared Lives or other suitably qualified statutory agencies, a registered charity or other third sector providers.

The membership pack will include the referral form, risk assessment, code of conduct, photography consent, Self-Medication Risk Assessment, these must be completed by the appropriate professional or service manager and the code of conduct to be completed by yourself.

The referral must also state whether the individual will be accessing the service supported or unsupported.

Without all correct documentation membership can not be processed until fully completed.
